How do I consent?

You can choose whether or not to share your information using JUYI.

We hope everyone will allow us to make key parts of their health and social care information available to NHS clinicians locally and social care professionals in Gloucestershire.

If you were registered with a Gloucestershire GP before March 2016 you will have received a letter asking if you want to share your information using JUYI.

Otherwise, when you registered with your GP practice, you will have been asked to opt-in or opt-out of sharing your information through JUYI.

If you are unsure about whether you have a JUYI record, ask your GP practice.

It is possible to change your mind at any time by speaking to your GP practice or by printing and completing the JUYI permissions form using the link below and returning it to your GP practice.

Healthcare staff will ask for your permission when they need to view your JUYI record, however there are circumstances where the care professional looking after you will not need to check with you before accessing your information, for example if you are unconscious or if you are not present.
